การเสริมจมูกแบบโอเพ่นคืออะไร?
การเสริมจมูกแบบโอเพ่น หรือชื่อเต็ม ๆ คือ “Open Rhinoplasty” เป็นการใช้เทคนิค Nose Reconstruction พูดง่าย ๆ ก็คือ การสร้างใหม่เพื่อปรับโครงสร้างจมูก โดยจะไม่ใช้ซิลิโคนในการเสริม ซึ่งจะสามารถที่จะปรับโครงสร้างจมูกเป็นรูปแบบใดก็ได้ตามความต้องการ ทำให้จมูกสวยได้รูปและดูเป็นธรรมชาติมากขึ้นนั่นเอง ทางแพทย์จะใช้เทคนิคโดยการผ่าตัดเปิดแผลบริเวณฐานของจมูก เพื่อที่จะสามารถตกแต่งกระดูกที่ฐานนั้นให้เล็กลงได้ จากนั้นจะทำการปรับรูปร่างปีกจมูกให้สวยงามและเข้ารูปมากขึ้น รวมถึงแต่งปลายจมูกให้ดูเรียวเล็กตามแบบสมัยนิยม และปรับให้เข้ากับโครงสร้างหน้าของแต่ละคน
ความพิเศษของการเสริมจมูกแบบโอเพ่นที่ได้กล่าวไปในข้างต้น คือการเสริมจมูกโดยไม่ต้องใช้ซิลิโคน ทำได้โดยการยืดปลายจมูกให้โด่งและมีความพุ่งขึ้น ด้วยกระดูกอ่อนกลางจมูก ซึ่งกระดูกในส่วนนี้จะเป็นส่วนเดียวในร่างกายที่มีลักษณะที่แข็งแรง แบน และเรียบตรง แพทยจะทำการผ่าตัดเอากระดูกอ่อนที่ว่านี้ออกสัก 2/3 เพื่อไว้ช้สำหรับเสริมปลายจมูกให้ดูพุ่ง จึงเรียกได้ว่าไม่ได้ใช้ซิลิโคนมาเสริมเลย แต่ใช้วัสดุธรรมชาติจากตัวเราเอง แต่ทั้งนี้ ถ้าความสูงของสันจมูกไม่พอ แพทย์จะเลือกใช้วัสดุสังเคราะห์อย่าง Gore-Tex เพื่อเสริมเพิ่มความโด่ง ข้อดีของเจ้าวัสดุตัวนี้คือ จะไม่ทำให้เกิดพังผืดหุ้ม ไม่มีหินปูนมาเกาะจนเกิดปัญหาระยะยาวเหมือนวัสดุอื่น ๆ และคงตัว ไม่ยุบหรือเสื่อมสภาพ วิธีนี้เป็นวิธีที่ได้รับความนิยมมากในประเทศเกาหลี
จมูกแบบไหนสามารถเสริมจมูกแบบโอเพ่นได้?
แน่นอนว่าถ้าอยากให้ได้จมูกที่สวย เข้ากับหน้าเรา ก็ย่อมมีข้อจำกัดในการทำ ที่อาจจะไม่ใช่จมูกในทุกรูปทรงที่จะทำออกมาแล้วสวยดังใจ ซึ่งจมูกที่สามารถเสริมจมูกแบบโอเพ่นมีดังนี้
1. จมูกสั้นที่มีความแหงนขึ้น ด้วยความที่จมูกมีความสั้น จึงต้องทำให้จมูกยาวขึ้น โดยการนำกระดูกอ่อนมาต่อให้ยาวขึ้น แล้วจึงยืดผนังกั้นเพื่อให้จมูกยาวและมีความเรียวสวยงามมากขึ้น
2. จมูกงุ้ม หรือจมูกแบบเหยี่ยว ที่มีลักษณะปลายตรงและตกลงมา ปัญหานี้เกิดจากโครงสร้างกระดูก ดังนั้นหากจะแก้ทรงจมูกแบบนี้ จะต้องใช้เทคนิคเสริมจมูกแบบโอเพ่นเพื่อยืดและยกให้จมูกมีความพุ่งขึ้นและดูสวยงามได้รูปขึ้น
3. จมูกใหญ่ เนื้อจมูกเยอะ เป็นลักษณะที่จำเป็นต้องลดขนาดของจมูกลงเพื่อให้มีความเรียวและเล็กขึ้น ซึ่งวิธีเสริมจมูกแบบโอเพ่นจะไปปรับโครงสร้างจมูก ช่วยยืดให้จมูกเรียว พุ่งสวย หมดปัญหาปลายจมูกทะลุด้วยการเสริมจมูกแบบโอเพ่น
บางคนที่มีเนื้อจมูกหรือเคยเสริมจมูกแบบซิลิโคนมักจะพบกับปํญหาปลายจมูกทะลุ ดังนั้น เทคนิคการเสริมจมูกแบบโอเพ่นจะช่วยแก้ปัญหานั้นอย่างถาวร นอกจากนี้ ยังสามารถไปปรับโครงสร้างจมูกทั้งหมดให้ดูสวยขึ้นได้ ไม่ใช่แค่เพียงให้จมูกดูเรียวอย่างเดียวเท่านั้น แต่รูปทรงที่ดูเรียวขึ้นนั้นมาจากการปรับแก้โครงสร้างจากภายใน ซึ่งโดยมากแทบไม่ต้องตัดปีกจมูกร่วมด้วยเลยเนื่องจากรูจมูกและปีกจมูกเรียวขึ้นประหนึ่งแล้ว ซึ่งทั้งนี้ จะขึ้นอยู่กับดุลพินิจของแพทย์ด้วย
ข้อจำกัดของการเสริมจมูกแบบโอเพ่น
การเสริมจมูกแบบโอเพ่นนั้นถือว่าเป็นการผ่าตัดที่มีความซับซ้อนและยากกว่าการผ่าตัดด้วยเทคนิคอื่น ๆ ดังนั้นจึงต้องเลือกคลินิกที่มีประสบการณ์สูงและเชื่อถือได้ รวมทั้ง การเสริมจมูกแบบนี้นั้นจะมีเรื่องของแผลตรงกลางจมูกเล็กน้อย ซึ่งจะกินระยะเวลาประมาณ 3 – 6 เดือน ถึงจะหาย อีกทั้งมีราคาแพง จึงควรหมั่นดูแลให้ดี เพื่อเป็นการเฝ้าระวังภาวะแทรกซ้อนที่อาจจะเกิดขึ้นได้
การเตรียมตัวก่อนการผ่าตัดเสริมจมูก
1. แจ้งรายละเอียดต่าง ๆ เช่น โรคประจำตัว ยาประจำตัว ประวัติการแพ้ยา ประวัติการผ่าตัด ให้แพทย์ทราบก่อนการผ่าตัด
2. ควรแจ้งให้แพทย์ทราบก่อนการผ่าตัด หากคุณทานยาในกลุ่มของแอสไพริน เช่น ยาแก้ปวด NSAID หรือยาในกลุ่มละลายลิ่มเลือดและยาที่มีผลต่อการแข็งตัวของเลือด
3. งดทานวิตามินและอาหารเสริมทุกชนิด เช่น วิตามินอี ใบแปะก๊วย น้ำมันปลา อย่างน้อย 1 เดือน
4. ควรสระผมให้เรียบร้อยก่อนการผ่าตัดและงดการแต่งหน้า รวมถึงงดการใส่คอนแทคเลนส์ในวันผ่าตัด
การดูแลตนเองหลังการผ่าตัด
1. ใช้สูตร 3:3 ในการบรรเทาอาการบวมและปวดจากแผลผ่าตัด คือ ประคบเย็น 3 วันแรก หลังจากนั้นประคบร้อน 3 วันหลัง
2. หลังผ่าตัด 24 – 48 ชั่วโมง อาจมีอาการปวดศีรษะ จมูกหรือบริเวณโครงสร้างใบหน้า ให้ทานยาแก้ปวดและยาแก้อักเสบตามที่แพทย์สั่ง
3. งดกิจกรรมบางอย่างที่ต้องออกกำลังมาก เช่น การวิ่ง การมีเพศสัมพันธ์ ซึ่งสามารถทำได้หลังอาทิตย์ที่ 3 เป็นต้นไป
4. นอนหนุนหมอนให้สูงขึ้น เป็นระยะเวลาอย่างน้อย 2 อาทิตย์
5. หมั่นทำความสะอาดแผล เช้า – เย็น และห้ามให้แผลถูกน้ำ เพราะอาจเกิดการติดเชื้อได้
6. ในช่วง 1 – 3 อาทิตย์แรก ควรหลีกเลี่ยงการสั่งน้ำมูก ขยี้จมูก ก้มหน้านาน ๆ และการยกของหนัก
